What is the TDM Exam?
The term therapeutics is defined as the application of knowledge in the pharmacological and non-pharmacological management of medical conditions. The TDM Examination aims to assess the competence of candidates at the level required of a family physician practicing independently and safely in Canada. It consists of patient-based scenarios representing common or important clinical presentations and problems that family physicians practicing in Canada should be able to competently manage in the following dimensions of medical care: health promotion and illness prevention, acute illness, chronic illness, and safety and adverse effects.
The examination is comprised of 40 cases with approximately 100 short answer write-in questions. Candidates are allowed up to three hours to complete the examination.

Over the duration of the course, students will review approximately 100 in-depth therapeutic drug monitoring cases that reflect the content and difficulty level of the actual exam.
For each case, the instructor will provide a clinical vignette with details about the patient’s medical history, and current condition, and Students will use this information to make calculations and determinations about the proper drug dose, monitoring parameters, and dosage adjustments for the patient. Cases will cover a wide range of medications that require therapeutic drug monitoring such as antibiotics, anticonvulsants, antiarrhythmics, and more.
In addition to working through individual cases, the course will also focus on building core skills for TDM including:
- Developing initial dosing regimens and adjustments
- Assessing the need for concentration monitoring
- Evaluating drug interactions and Special patient factors
- Applying pharmacogenomic concepts
- Identifying adverse effects and toxicities
- Using population models for dose individualization
The goal of the course is to fully prepare students for every aspect of the TDM questions they will encounter on the Ontario Ready exam. The exposure to approximately 100 diverse cases will build proficiency and decision-making to help students optimize drug therapy in preparation for clinical practice.
